U.S. Rowing Liability Waiver Form

Sign the form online by following these instructions:

1. Go to http://www.rowingwaiver.com
2. Look for the blue arrow & click on the circle marked "Sign a Waiver".
3. Choose an option: "Junior Male" or "Junior Female"
4. Fill out the Contact Information and click on "Continue".
5. Fill out the Entry Information: The affiliation is City Honors High School and the team representative email address is typecerg@msn.com. The team representative is Berdzik T. Click on "Continue".
6. Fill out the Parent/Guardian Information. Click on "Continue".
7. Review the waiver and put a check mark in the box at the bottom. Click on "Continue".

You are done!

City Honors and West Side Forms

The following forms can be opened in MS Word. Please print these forms, fill them out, and turn them in at a parent meeting or give them to your coach. The medical form will be kept confidential to be used only in case of emergency. However, please make sure your coach knows about any medical issue that might affect your rowing.
